A 50 year old male came with complaints of diminished vision in right eye for past 3 weeks. He was a known case of diabetes, hypertension, hyperlipidemia and coronary artery disease with history of significant stenosis of bilateral carotids (> 80% stenosis in right carotids and > 50% stenosis). best corrected visual acuity of right eye (OD) was hand movements and left eye OS was 6/9. Intraocular pressure of OD was 7 mm Hg and OS was 12 mm Hg. Anterior segment examination of OD revealed Neovascularisation of iris and posterior chamber intraocular lens. Left eye was normal. Fundus examination of OD revealed small dot hemorrhages in the MID periphery and OS was within normal limits. Fundus fluorescein angiography was done which showed normal choroidal filling and delayed retinal arterial filling at 1 minute 5 seconds in the OD. OS was normal in FFA. The fundus camera was focused on the iris during angiography and we were able to capture the iris vessels filled with the fluorescein dye [Figure 1] as depicted in the picture. Patient was advised intravitreal injection ranibizumab with pan retinal photocoagulation for the right eye and underwent the same.{Figure 1}
